1. A list of courses, internships, research or creative projects, or work experiences that have provided useful background for your project.
Please list your experiences in order of importance beginning with the most significant.

2. Please provide an abstract or brief description of your proposed project.  You should include a clear statement of the project focus, thesis, or central argument, an indication of how your plan to develop that focus, and a statement of project's significance. (approx. 200 words)

3.   Personal Statement (approx. 1000 words)
Please describe your academic and other interests and aspirations.  Your statement should detail a specific area of proposed research, your reasons for wanting to undertake this project, and how the project will contribute to your overall academic or intellectual development.  If you have been engaged in an activity or research or creative project that has been important in clarifying or strengtehning your interest in your field of study, please detail that activity or projects.


4. Mentor / Reference
Please provide the name, department and email address of the faculty member who will be providing your letter of recommendation. Most often, the faculty member who will serve as mentor provides the letter of recommendation.  However, if the faculty member providing your letter of recommendation is not the individual who will be serving as mentor, please provide both your mentor's and recommender's information.
