Do you need a job physical, a driver’s license, or a health form filled out for a program? You can get your physical exam at Student Health Services. When you make an appointment, please tell the receptionist that you have a form to fill out and bring your form with you to the appointment.
Please read your forms before you come to your appointment. Many forms require you to complete parts yourself before the clinician can sign them. Others require immunization dates.
NOTE: There is a charge of $25 for physical exams that require the clinician to fill out a form.
Note: some forms require TB (tuberculosis) testing. We provide TB testing on Monday, Tuesday, Wednesday and Friday only.
Planning a trip abroad? Need information, medication or vaccinations for your trip?
We provide travel consultations and recommend vaccinations and medicines you may need. Please schedule your appointment as far in advance as possible. Most vaccines take up to four weeks to start protecting you, and some shots have to be given six months before you travel. For more information, go to:
