Starting in JUNE 2013, Students 18 years old and over can complete the form by logging into My Online Student Health found on the right hand side of this page and clicking on the Meningitis Form link.
Students can also complete a paper form. Students under 18 must have a parent or guardian complete the Meningitis Form for them and send it to:

In addition, Student Health Services recommends, but does not require that students receive the following vaccinations: Measles/Mumps/Rubella, Varicella, Hepatitis B, Hepatitis A, Influenza and Tetanus-Diptheria.

There are ONLY TWO mandatory Student Health requirements for certain groups of
students:
1.) Students who will be living in the residence halls are required to have the meningitis vaccine. The vaccine can be
declined, but the student has to complete an online certification form.
Students living in the residence halls who are over the age of 18 will
receive e-mails starting in June with instructions to complete an online form. The
parents/guardians of students under 18 will receive a letter and a
paper form in June. Students or their parents are required to either
certify that they received the Meningitis vaccine or sign a waiver
declining the vaccine before they will be allowed to move into the
residence halls. The waiver can be signed now and you can then receive
the vaccine in Student Health after you come to campus.
2.) The second mandatory requirement is for students who are citizens of
countries with a high rate of tuberculosis, including China. These students are required to be screened for tuberculosis.
Students required to have the tuberculosis screening will receive
information in early September and that testing must be done in
Student Health. We do recommend other immunizations but they are only
recommended not required.
No other forms or physicals are required by Student Health.
