Certificate of Insurance

All requests for evidence of insurance, either certificates of insurance or letters evidencing coverage, shall be forwarded to the Director of Risk Management and Insurance. Along with the written request, there should be a copy of the contract/agreement that requires evidence of insurance and a completed certificate request form. Certificates of Insurance will be processed by the Risk Management and Insurance Department within 48 hours of receipt.
