University Provided Enrollment Verification

If Enrollment Verification through TUportal is insufficient, please fill out an Enrollment Verification Form online and then print and return it (in person or by mail) to:

You should provide the following information and allow a minimum of two weeks for the release of enrollment verification letters and forms:

your printed name and SIGNATURE
social security number or TUid
information such as dates of attendance, date of graduation, program of study
names and addresses of recipients and number of copies needed
your daytime phone number & email address

You should also include any forms provided to you by the agency, otherwise, we will send our standard University letter. Please click on the link above for a copy of an Enrollment Verification Request form.
