Please print and complete these forms.  When you have completed them, please return them to the front desk at the IELP office during walk-in hours.    PLEASE DO NOT SUBMIT TWO COPIES OF YOUR CEP FORM.  If you have already given Dennis a CEP choice sheet, you do NOT need to give our office another copy.
