To be considered, applicants must have:

  • A Doctor of Pharmacy degree from an ACPE-accredited school or college of pharmacy
  • Completed or in the process of completing an ASHP-accredited Pharmacy Practice Residency or possess equivalent experience
  • Agreed to participate in the ASHP/NMS Pharmacy Residency Match


Application requirements:

  • Letter of intent that describes your experiences to-date and your desire to practice in infectious diseases
  • Curriculum vitae
  • Official transcripts from post-secondary institutions attended
  • Three letters of recommendation

 

The above documents may either emailed or mailed as hard copies, but email is preferred for all but transcripts.
