Cancellation Policy

There is a 48 hr cancellation policy for ALL FACS work scheduled. You are responsible for paying for all time reserved unless changed more than 48 hrs prior to the reservation. This policy has been in effect since 04/12/06

Requisitions

An approved purchase requisition must be on file with the billing administrator prior to appointments being scheduled. All purchase requisitions expire on June 30th, and must be renewed annually.
