OIS as U. S. Passport Acceptance Agent

The Office of International Services (OIS) serves as a U. S. Passport Acceptance Agent, a satellite office of the Philadelphia Passport Agency, by appointment only.  Appointments can be made to see the Passport Acceptance Agent by calling the Office of International Services at (215) 204-7708. 

APPLICATION FORMS

All U. S. passport application forms are available online at http://travel.state.gov/passport/forms/forms_847.html and should be downloaded and completed, but not signed, before meeting with the Passport Acceptance Agent.

MATERIALS REQUIRED

The following is an abbreviated list of required materials:

• Proof of U. S. citizenship
• Proof of identify
• Two (2) acceptable passport photographs
• Fees (check or money order only) listed below

FEES REQUIRED

There is no discount for Temple employees or students. 

You must submit the required, current fees by CHECK OR MONEY ORDER ONLY; cash and credit cards cannot be accepted by OIS (you may check the US Passport Office web site at http://travel.state.gov/passport/get/fees/fees_837.html to confirm current fees):

For applicants age 16 or over

•  Passport Fee, $75.00 (check or money order payable to U. S. Department of State)

•  Execution Fee, $25.00  (check or money order payable to Temple University ) 

For applicants age 15 or under

•  Passport Fee, $60.00 (check or money order payable to U. S. Department of State)

•  Execution Fee, $25.00  (check or money order payable to Temple University )

EXPEDITING THE PASSPORT APPLICATION
