Sponsored Projects Administration Form (SPAF) - This form is submitted with the initial grant application to Sponsored Programs or to OCT for your Clinical Trial Contract Completion.
Research Grant Registration - (Hospital) Account Request - This form is used to establish a financial account for services within the hospital that will be billed to the grant and not to a patient's insurance.
Research Grant Registration - Participant Form - This form is used when you have a research patient registering for a test that should be billed to the grant account
Subject Reimbursement Check Request Form - This is the policy and forms to be adhered to when compensating study participants by check.
