Online CME Transcripts
CME transcripts are available for individuals who have attended Temple-sponsored Continuing Medical Education Programs that have occurred since 2003. To obtain your Temple CME transcript, please follow the appropriate set of instructions located below.
If you are trying to obtain your undergraduate or graduate transcript from Temple University, contact the Office of Academic Records by telephone at 215-204-1131 or obtain information and a request form online by going to:
If you already have a user name and password:

Enter your user name and password and click "Log In" |
3. |
Under "Transcript Lookup", select a date range and click "Get Transcript" to get a transcript for the selected date range |

Click "Get Transcript" while the Start Date and End Date are blank to get a transcript containing all your registered courses |

Click the link that appears to view the requested transcript |
If you do not have a user name and password:

Click "Set Up User Name and Password" from the list of available subevents |
3. |
Click "Add to Registrations" button |
4. |
Click "Register" button |
5. |
Enter your personal information and a user name and password and click the "Continue" button |
6. |
Click "Complete Registration Process" button |
7. |
Click "Print Transcript" button |
8. |
Enter user name and password and click "Log In" |
9. |
Under "Transcript Lookup", select a date range and click "Get Transcript" to get a transcript for the selected date range |

Click "Get Transcript" while the Start Date and End Date are blank to get a transcript containing all your registered courses |
