Transcripts and Records

The Office of Student Records (OSR) is responsible for maintenance of academic records for current medical students and medical alumni. Services for enrolled students include matriculation verifications, processing of loan deferment forms, replacement of missing photo identification cards, assignment of campus mailboxes, and transmittal of official documents.

 

Graduates of the medical school should contact the office for assistance in processing enrollment verifications, transcripts and other materials required for licensure or employment.

 

Transcripts and Document Requests

 

Transcripts, certifications and other official documents for medical school students and graduates are supplied by the Office of Student Records. There is a $6 fee for each transcript requested. Forms, verifications and MSPE’s (Dean’s Letter) are processed free of charge.

As official certified transcripts are sent to third parties only, be sure to indicate the name and address of the state agency, hospital, school or board where it should be sent. Transcripts mailed directly to alumni are marked “student copy” and are considered unofficial documentation. If you require more than three transcripts, please provide labels that can be affixed to envelopes.

 

Mail or fax your Document Request Form (federal law requires signature) to the address below. If applicable, enclose a check or money order payable to “Temple University School of Medicine.” Credit cards and cash are not accepted.

 

Requests may also be dropped off at the Office of Student Records in the Medical Research Building, Room 106, Monday through Friday, 8 am to 5 pm.

 

Requests will be processed within 10 business days after the letter and payment are received. The Student Records Office does not fax transcripts, verifications or MSPE’s to the requestor or a third party. Transcripts cannot be issued if there are holds on the academic record.
