All registration fees include conference materials, refreshments during breaks, lunches and conference dinner/reception.

Registration fees can be paid by credit card or cash. Please select the payment method you will use.

Cash
  Payment, in United States Dollars only, can be made in cash on arrival at the conference. Do not send cash by post.

If you elect to pay the registration fee in cash at the conference please still submit this online form so we can have conference materials ready for you on arrival.

Notes

All registrants will receive an email confirmation that they have been registered for the conference following receipt of this form (online or a mailed hard copy) and their payment.

All registrants will receive a hard copy receipt (proof of payment) when they check in at the beginning of the conference (or before if requested).

Fifty percent of the fees will be refunded only if written request of cancellation is made 10 working days prior to the conference, i.e., by August 14. No refund can be made for cancellation after this date.

If you are prevented from attending the conference, you may pass on your registration to a colleague (if this should happen please please notify Cheryl Bracken).

You may want to print a copy of this completed form for your records before submitting your registration.
