About the Transitions Project

The Transitions in Aging project is funded by the Pennsylvania Developmental Disabilities Council.

The modules were developed by Dr. Pei-Chun Hsieh, a faculty member in the Department of Rehabilitation Sciences at Temple University, with support from Ms. Celia Feinstein, the Executive Director of the Institute on Disabilities and her staff at Temple University.

We would also like to thank Ms. Cassandra Watts, and Kendra Smith for their assistance with this project.

How to Navigate

Below there are nine modules (online trainings) containing information on topics related to successful aging for individuals with developmental disabilities and their families. Three provider modules are developed to inform people who are working with individuals with developmental disabilities on how to assist their consumers age well. Five consumer modules focus on the strategies for individuals with developmental disabilities and family members to live in their prefered environment as long as they wish.

As you scroll down the page, the three provider modules will appear first, followed by the five consumer modules. You can review the topic of each module and its learning outcomes to determine what information you will take away after completing the module.

To enter a module: Click on "Begin this module" listed under the outcomes. This will take you to the main screen of that module.

We understand people learn differently. You can choose to read the information in the modules or you can click on the audio option to have the information read to you.


Provider Modules

Aging is a natural process associating with changes in all aspects of our life. These modules are designed for service providers, care staff, family members and others to promote a better understanding of the aging process among individuals with developmental disabilities. We hope that with a collective effort, aging individuals with developmental disabilities can lead a healthy and independent life in the community.


Consumer Modules

When we get older, we may experience some changes in our body, which may affect our ability to do the activities we love. These modules are designed for individuals with developmental disabilities to learn about how aging may affect us and how to plan for these changes. These modules discuss different topics, including how to stay at our home as long as possible, how to be an active community member, and how to live a healthy and independent life as we age.
