| The Total Wellness rewards program provides you with tools, resources and challenging activities to help you take steps to improve and maintain your overall health and well being. This program provides individual preventative care recommendations, an on-line health assistant, team challenges, wellness seminars and educational resources. You will earn rewards based on points you receive by participating in activities, team challenges and completing milestones. There are two tier levels to the program. |

| Employees enrolled in one of the Universities health insurance plans, Personal Choice or Keystone Health Plan East. |

| $50 gift cards, maximum gift card value per plan year per employee - $200. |
