Learning Coach Commitment Form

Time Line for Submission of Forms
This is an overview of the steps a Lead Agency takes in order to start an SFW class. The document includes a list of the forms to be submitted to Temple University Harrisburg, with information regarding when each set of documents are to be submitted.
Notice to Start (Approved Provider)
Prior to a class beginning, the Notice to Start form should be sent from the Approved Provider to the Lead Agency or to Temple if there is no statewide agency. The form should be submitted 30 – 90 days prior to class beginning. The Lead Agency may charge a fee to the Approved Provider which may be collected at this time to cover initial administrative costs. The Approved Provider is also to supply the other documents requested at the bottom of the Notice-to-Start form to either the Lead Agency or to Temple. *Note that if any Instructors or Learning Coaches change, the Approved Provider will submit a revised and corrected form to the Lead Agency or to Temple. National information contained in the requested documents is used to contribute to a national database maintained by Temple. This database will be made available to members of our National Family Development program.
Notice to Start (Lead Agency)
Prior to a class beginning, the Notice to Start form and accompanying required documents can be sent from Lead Agency to Andrea Warner, Temple University Harrisburg. The Lead Agency has the option to instead send a copy of the Approved Provider Notice to Start it has received from the Approved Provider. The form should be submitted at least 30 days prior to class beginning. *Note that if any Instructors or Learning Coaches change, the Lead Agency will submit a revised and corrected form to Temple University Harrisburg.
Attendance and Participation Guidelines
This document outlines attendance and participation guidelines that can be used as an example and modified to fit unique needs. Approved Providers are bound to the SFW policy found in the Policies and Procedures document. The Lead Agency may impose additional guidelines that are more strict than those contained in the policy.
Request for Exam and Portfolio Review
This form is used to request the exam and to submit the worker portfolios. To ensure your requested exam date, this form and the worker portfolios must be received by Temple University Harrisburg 15 days prior to the exam date.
Final Feedback Form
The Final Feedback completed by students on the final day of class and usually submitted to Temple University Harrisburg or to the State Lead Agency with the portfolios in accordance with the terms set in the MOA.
Final Participant List
A hard copy of the Final Participant List should be sent along with the portfolios to the State Lead Agency or Temple University Harrisburg in accordance with the MOA. It serves as the final list of participants who have completed the classroom training and will be taking the exam. The Final Participant List should also be emailed to firstname.lastname@example.org.
Daily Attendance Sign In Sheet Template
This form can be used to track student attendance according to the Class Session Attendance Guidelines. Students are to sign in for each session including upon return from lunch and breaks.

Directions for Transcript Request
Please review these instructions prior to completing the transcript request forms in order ensure your request will be processed! You will be submitting two forms and payment to Temple University (fax to Andrea Warner at Temple University Harrisburg 717-221-1634), who will forward to Excesior College for transcript.
Excelsior Transcript Service Application Form
This is the first of the two forms that must be completed and forwarded to Temple in order for SFW graduates to receive a transcript showing 7 hours of credit from Excelsior College for completing the SFW course.
Excelsior Official Transcript Request Form
This is the second of two forms that must be completed and forwarded to Temple University Harrisburg in order for SFW graduates to receive official transcript that can be sent to your college or employer. You must complete one form for EACH official transcript you request.
