Forms
Students are encouraged to download forms that are often helpful in requesting services at DRS. The following forms are offered for your convenience:
- Student Intake Data Students are asked to complete this form prior to meeting with a DRS advisor for the first time.
- Test Administration Form for MAIN This is the DRS "Blue Form." Students are encouraged to download and begin to complete this form when they plan to request test proctoring services at DRS. Students should take the partially completed form to their instructors for input before submitting to DRS. Forms must be submitted three (3) days in advance of your exam.
- Test Administration Form for AMBLER This is the DRS "Blue Form" for Temple University Ambler. Students are encouraged to download and begin to complete this form when they plan to request test proctoring services from DRS. Students should take the partially completed form to their instructors for input before submitting to DRS. Forms must be submitted three (3) days in advance of your exam.
- Test Proctoring Agreement for Main Campus This form explains the policies and procedures associated with taking exams at DRS. Students are encouraged to carefully review this form and should submit the completed agreement to DRS prior to taking their first exam.
- Reader Service Request Form Students requesting texts on CD or audio-tape are requested to complete this form and submit to DRS Student Service Coordinator.
- Notetaker Packet (pdf) Students with disabilities approved for requesting volunteer note takers may download this packet.
- Volunteer Notetaker Form (pdf) Students interesting in volunteering to take notes for a classmate should complete this form and submit it to DRS to obtain Student Development Transcript recognition.
- Charlotte W. Newcombe Foundation Grant: Students in need of disability-related grants or scholarships may submit an application that is due the 3rd Friday of each month. (Please note that no applications will be processed in December or June).
