Forms

To request telephone or Internet service, please fill out the appropriate form and submit it to the Office of Telecommunications.

Many Telecommunications forms are available for electronic submission. Simply click on the Internet Explorer icon next to the form title, provide the required information, and select Submit. Your request will be delivered electronically to the Office of Telecommunications.

Departmental Forms

With the exception of the Request for Cellular Equipment and Service and the Customer Service Order Request forms, these forms are available to fill out and submit online. Departmental forms are also available in Excel, Word, or PDF format.

Cellular Equipment & Service Request Form ()
Facilitates requests for cellular service, such as commencing new service, transferring existing service, deactivating/suspending service, and making plan or equipment modifications.

Customer Service Order Request ( or )
Facilitates the installation, move, and removal of telephone and Internet wiring, as well as other miscellaneous requests.

Facilitates the assignment of an IP Address to allow for Internet connectivity.

Residential Student Forms

Student Billing Claim Request ( or or )
Facilitates the investigation into telephone and Internet charge inquiries made by students living in campus housing.

Disconnect Request
Facilitates the termination of telephone service. To access this request form, log in to myhousing.temple.edu and click on Telephone & Internet Access.
