Subject: Social Security Number Access Policy Reminder

This is a reminder that anyone who accesses Social Security numbers must have an Access to Social Security Number Approval form signed and on file with the University Privacy Officer. Without a signed form on file, access to Social Security numbers is strictly forbidden.

Internal Audits and Information Security will be checking accounts of those who have access to Social Security Numbers to ensure that a form is on file. If we discover that a form is not on file, Information Security will terminate the user’s ability to access Social Security numbers until a form is on file with the University Privacy Officer.

To obtain an Access to Social Security Number Approval form, please download the document from our Web site at http://ssn2tuid.temple.edu/SSNScreenAccessRequest.pdf. The form requires signatures from the requestor, the requestor’s supervisor, the requestor’s Vice President or Provost, and the Vice president for Computer and Information Services. Once the form has the appropriate signatures, it should be sent to my office, at 705 Conwell Hall.

It is important that each department head or supervisor disseminates this information down to their employees as a form is required for each individual who needs access to Social Security numbers.
