Equipment Request Form

Please note: CTS Does not have or create connect accounts. Please submit a ticket at tuhelp.temple.edu. Delivery of portable equipment is limited to twice a semester at night, unless a smartroom is NOT available and staffing IS available.

This form requires at least two days advanced notice. Otherwise, you will need to call or visit the appropriate CTS location.
