office at 204-7174 and request a form for a Courtesy Card. Human Resources
must authorize the request.
If you are a Health System employee then you may complete the Courtesy
Card request form, have your department head sign the form and send
the form to Administrative Computer Services.

on or near campus?
The Diamond Dollars office maintains one wireless reader for special
events. Contact the Diamond Dollars office for availability and procedures
for scheduling usage.

entrance?
Requests for new security reader installations begin with a Work Order
Request forwarded to the Office of Facilities Management. They will
perform a site feasibility analysis with projected costs for the installation.
Once you agree to move forward with the project, a service request needs
to be submitted to the following departments to support the project
tasks: Administrative Computer Services to program the reader and cardholders;
Office of Telecommunications to provide network connectivity to the
security reader. Contact Administrative Computer Services with any questions
about this process.

New location requests for Diamond Dollars are processed by the Diamond
Dollars office, who will instruct you on the procedures for this project.
