Therapist – Acute Partial Hospital program
within a partial Hospital program- Main Center
Provided direct therapeutic services,
including individual counseling, and a variety of group therapies in an
intensive acute Partial Hospital Setting. Functions as a member of a clinical
team, maintaining clinical documentation for a primary caseload including
daily progress notes, periodic treatment plans, discharge/transfer
summaries, and intake assessments. Assists psychiatrics with medication
monitoring of Partial Hospital clients including educating clients as to
following prescribed medication regimen. Communicates medication issues to
psychiatrists and partial hospital staff. Acts as liaison to other agencies,
when appropriate, to aide in establishing linkages to other services for
clients.
