Students, faculty and staff in the CIS Department may apply for a personal account to use with CIS department computers. Application forms are available at the CIS Department Laboratory offices, rooms 201 and 205 Wachman Hall. Copies of the forms are also available online for printing.
Two forms must be submitted in order to receive a personal account. One is the CIS Department form, and the other is the Temple University Policy and Procedures form. Once BOTH of these forms have been completed, they may be brought to the CIS Department labs offices for processing. There is someone on duty to answer questions and resolve issues. After the information on the personal account forms has been verified, a personal account will be created for the applicant. For the account setup procedure to be completed accurately and quickly, we urge all applicants to PLEASE fill out these forms CAREFULLY. Forms that are not completly filled out, or have information that cannot be understood, will in delays in account setup.
Undergraduate students will need a faculty sponser to sign the personal account form. Graduate students do not need a faculty sponser. A reminder that BOTH forms need to be signed by applicants applying for personal accounts.
Temple University and the CIS Department provides the CIS faculty students, staff and administration with the necessary computers and resources for education, instruction and research. Accounts that allow access to these computers and resources are a PRIVILEGE, and should not be taken for granted or abused. This privilege may be revoked for breach of the Temple University Academic Computer Usage Policy ("The Computer Usage Policy"). Applicants that are applying for a personal account should read the policy carefully, and make sure that they understand and are willing to follow them. Upon signing this policy form the applicant has agreed to follow the rules stated in the policy.
Violations of the policies set forth in these documents may result in suspension, or revocation of personal computer accounts and other computer privileges. In some situations disciplinary action maybe taken as described in the "Student Code of Conduct and Disciplinary Procedures". Disciplinary procedures of Temple University under the relevant policies for faculty, staff, administration and students do apply. In the worse case senario civil or criminal prosecution under federal and/or state law. Penalties under such laws could include fines, orders of restitution, or in the most extreme cases imprisonment. These policies are there to protect users, and the computer environment.
After a personal account is setup, the user will be issued a login name and password. This account information may only be obtained by the applicant, in person, from a staff person in the CIS Labs offices on the second floor of Wachman Hall. Personal accounts are NOT to be shared with other people. Such sharing will cause at least a suspension of the account. Remember to completly logout of your account when your are finished using computer resources. Report any problems with your personal account to the system administrators as soon as possible. Do not leave login information where it is easily available; protect it like you would any important personal information.
