Application Instructions for the Doctorate in Occupational Therapy (DOT)

1) Application:

You will need to complete the application form either online (http://webserv.adminsvc.temple.edu/GradApp/accountadmin.asp)
or print it out and mail it to:

Follow the Application Instructions to complete the two page application form. These can be obtained by going to http://www.temple.edu/grad//admissions/documents/

You should include a statement of professional plans and goals on a separate but attached document as indicated in item 22 on the application form. You can also include a separate statement of exception circumstances as indicated in item number 23 of the application form only if appropriate. For item 14 on the application form, you should choose Health Sciences Campus.

A $50.00 non-refundable fee must accompany the application (you can send a check or money order with the mailed application or you can pay on-line if you completed the on-line application.)

2) Letters of Reference

At least 2 letters of reference on the “Reference Report for Graduate Study” form that can be found at:

You should complete Section I of each reference form and have the person providing the reference to complete Section II. After completion, these should be sealed and sent only by the person to the following address:

3) Academic Records:

All applicants must submit one certified copy of official transcripts from every college and university attended, including Temple University if you have completed coursework here. Temple University provides a request form that you can obtain for this purpose which can be found at: http://webserv.adminsvc.temple.edu/GradApp/popDownloadforms.asp

Complete section one of this form and mail/fax it to the Registrar’s office at the colleges or universities that you have attended requesting the completion of part two. Request that the university return it in a sealed envelope bearing your official transcript to:

4) Standardized exams:

You are only required to submit GRE scores if you have below a 3.0 in your cumulative GPA in your Master Degree program. GRE scores will be accepted if you have taken the tests within the past five years and scored a percentile of 50% or higher in one of the sections. If you were not required to take the GRE for your Master Degree program and do not have a 3.0 in your cumulative GPA in your Master Degree, you can find information on the test at:

Additional Information:

• Information on financial assistance is provided on page three of the application instructions. General information on tuition and fees are located on page 4 of this document.

• We encourage you to submit your application by 12/1 for spring admission and 8/1 for fall admission.

• We will contact you regarding an interview to complete your application process. Interviews are completed in two formats including face to face or over the phone depending on the location of the student.

Admission Requirements:

Applicants must be certified as Occupational Therapists via the procedures specified by the NBCOT and have a Master Degree in occupational therapy or another field. Applicants are expected to have a cumulative GPA of 3.0 in graduate work and/or a GRE score of 50% or higher. GRE scores are not required for the application process if you have successfully completed graduate work with a GPA of 3.0 or above.
